Nexyx, by Prognostyx

The layer beneath care.

Not another record. Not another workflow. Nexyx sits under both: a health record the patient owns, an inbox that shows a care team only what needs them, and Prognostyx watching for the signs that something is about to go wrong.

In development · starting with dialysis access

Why this exists

The warning signs are there. Nobody sees all of them.

For someone on dialysis, the access is a lifeline. When it starts to fail, the clues usually show up first as small things: a number that drifts, a nurse who notices a change, a photo, a missed visit.

Each clue lives somewhere different. The dialysis center has one, the surgeon another, the patient a third. By the time they are put together, the easy moment to act has often passed.

We are building the place where those pieces meet, and something that watches them together.

What Nexyx is

Three ideas, one record.

01

The patient owns it

Your record belongs to you. Add to it, see everything in it, and decide exactly who else can: your doctors, your dialysis center, the family member who drives you there.

02

The team sees what needs them

An inbox of patients who need attention now, in order. Everything else is one tap away in a feed built for fast review. Each person chooses what reaches them.

03

Prognostyx watches for trouble

A predictive layer that looks across the whole picture and raises a hand early, to the care team and to the patient, in plain words. A clinician always makes the call.

For care teams

Start the day with what matters.

  • Patients who need you, most urgent first, with the action right there.
  • A nurse can escalate to a physician in one step, and it always gets through.
  • Nothing to fill in. What people do is recorded as they do it.
  • Share a case with a colleague by showing a code. They wait until you let them in.
Inbox · 3 patients need you

dialysis access · 9:14

Prognostyx: this access needs a look. The trend over recent sessions has changed.

Escalated by the dialysis nurse · “Feels different today.”

Patient B

Patient C

A result arrived, added by the patient from home.

Patients and families

Your health, in your hands.

One record that goes where you go. Snap a photo of a result and it is in. Hear about a concern at the same time your care team does, in words that make sense. Bring in the people who help you, and only as far as you choose.

You stay in control of who sees what.

Clinics and care teams

See it coming.

For dialysis centers, nephrology and vascular practices, and the health systems around them. Bring a whole patient list in at once, give every role the view that fits their job, and let the early warnings find the right person.

Tell us about your clinic and we will be in touch.

Who is building it

A surgeon who sees the failures. Engineers who predict them.

Gabriel Herscu, MD

Clinical lead

Gabriel is a board-certified vascular and general surgeon who has spent his career on the conditions Nexyx starts with. He is Chief of Vascular Surgery at Aria Vascular and at San Joaquin General Hospital, where he also directs the surgical residency program.

Elmer Tolentino

Sr. Distinguished Engineer, HPE

Elmer has spent three decades as a reliability engineer, building the discipline of knowing a system is about to fail before it does. He was a principal reliability engineer at Juniper Networks for more than twenty years and is now a Senior Distinguished Engineer at Hewlett Packard Enterprise.

Aleks Movsesyan

Engineering leader

Aleks leads engineering at Prognostyx, turning the clinical idea and the prediction engine into software that patients and care teams can rely on. He has spent his career building and running product engineering teams, most recently as VP of Engineering at The Mind Company and, before that, Head of Engineering at SendOwl.
